Here is fine and we apologize for this awful experience. As Gil says,

We do test our tubes rigorously, and we are using the best new production tubes that we can obtain in quantity. That being said, there are some of these tubes that have failed in the field.

Please follow these steps to get new tubes and fuses sent out to you: How can I make a replacement part request?
